Demographic Data Transmittal Form

Hiring units are required to submit this form to every  new employee at the time when the new employee begins initial employment with the University.   Providing the information as requested by the form is voluntary on the part of the new employee, and a refusal to provide the information should not subject the individual to any adverse treatment.  The information on the form is confidential, and the form should be destroyed by the hiring unit once the information is entered into the campus Payroll Personnel System.  

The data on the form is used for statistical purposes only.  The Office of Academic Diversity and Equal Opportunity, for example, utilizes the information to develop the campus academic affirmative action plan as required by  regulations governing federal contractors. 

The Demographic Data Transmittal form should NOT be used to collect gender and ethnic information from applicants; please us the Supplement to Application form for that purpose.
