To download a specific form, click the desired format link.  Under Netscape you will be asked to either "Save" or "Open" the file. Internet Explorer will automatically open the file in your web browser. You may save the file from your browser using File -> Save As.

All PDF (portable document format) files are created using Adobe Acrobat.  If you do not have the Acrobat Reader installed on your workstation, you can download and install the free Acrobat Reader software.
