COMPLIANCE REPORT
Introduction
The UCSD Academic Diversity and Equal Opportunity Compliance Report is used to
document the open recruitment of academic personnel other than
Ladder-Rank faculty on the General Campus and at SIO and all open recruitments at SOM. Submit the Compliance Report to the Office of
Academic Diversity and Equal Opportunity (OADEO), mail code 0029. Once the report has been reviewed,
OADEO will forward it to the appropriate Dean for approval. The hiring
unit will be faxed the approved report of which a copy must be included in
the appointment file sent to APO.

INSTRUCTIONS FOR COMPLETING A COMPLIANCE REPORT
Department/ORU:
Enter the name (not the number) of the department or ORU in
which the appointment will be made. Usually, but not always, this is the
department/ORU that did the recruitment.
OADEO Recruitment Plan Number:
Enter the number assigned by OADEO at the top right of the Recruitment Plan
Summary Form which was approved for the search. This is OADEO's tracking number
and allows the Compliance Report to be matched with the correct Recruitment
Plan on file.
Proposed Candidate:
Enter the name of the proposed candidate. A separate
Compliance Report must be completed for every proposed candidate.
Gender:
Enter "M" for Male or "F" for Female.
Ethnic Code:
The following codes are established to describe ethnic
categories:
- A - African-American
- E - Hispanic
- B - Asian/Pacific Islander
- C - American Indian/Alaskan Native
- F - Non-minority
- U - Unknown
Effective Dates:
Enter the anticipated begin date and end date of
appointment.
Total Applicant Pool:
An individual is to be considered an applicant if there is
clear evidence of interest in the position in some tangible form such as an
inquiry letter or a curriculum vita. Note that the first column is for the
total by gender followed by tallies of gender within each ethnicity. Please
check that columns and rows total correctly.
Number Seriously Considered:
This section should represent the selection cut that
narrows the pool to only those for whom more formal steps in the selection
process--such as interview, reference check, opportunity to provide a guest
seminar--are considered worthwhile to pursue.
Seriously Considered Applicants:
List the name, gender, and ethnicity of each Seriously
Considered Applicant. The list should reflect the numerical data in the
"Number Seriously Considered" table above.
Prepared by:
This is the staff person OADEO will contact if there are
questions or problems regarding the Compliance Report.
Organizational Unit Head (typed):
Please type name and position title of the individual
signing the Compliance Report. This should be the unit Chair or Director but
may be an Acting or Vice Chair/Director in the absence of the Chair/Director.
It may not be an individual with a Staff title, e.g., MSO.
Dean or Final Signature Authority (typed):
Please click on the box next to the words Name/Title
(typed): in the bottom signature area marked APPROVED: and choose the name
of your Dean or Final Signature Authority from the drop down list that
appears.
Recruitment Sources of Applicants:
List the name of the journal or other source from which
each applicant learned of the position. Complete a block for each
recruitment source even if the source did not generate any applicants; include
an "Unknown" source if appropriate. The
"Total" at the bottom should reflect the data in the "Total
Applicant Pool" table on Page 1.

- selection criteria
- selection process
- how each seriously considered candidate met or did not
meet the criteria.
- Chair Statement Guidelines
- Outreach Documentation - one copy of each published ad as it
appeared in a journal, sample letter(s) sent to colleagues/institutions
with list of addressees, flyers, e-mails, etc.
